CTRKids Family Information Update

As we prepare for the upcoming ministry year, we ask all families to complete this annual Information Update. Keeping this information current helps us communicate effectively with your family, plan CTRKids programs, and provide a safe and supportive environment for every child.

All families should complete sections 1 and 3. If your family is new to CTRKids or if any of your basic family or child information has changed since you last registered, please complete the applicable information sections as well.

Required fields are marked with an asterisk (*). Optional questions may be left blank if they do not apply to your family. For questions that only apply to new or updating families, you may skip them if your information is already current.
